The person behind the plan
State medical boards oversee doctors’ licences. Their public records show licence details and discipline, such as restrictions on a doctor’s work. What you can see differs by state (September 17, 2026).
- The person caring for me: complete name and job title.
- Link to the licence record and whether the licence is current.
- A restriction on their work or a note I need explained.
The proposed product
Get the wording clear enough to take to another conversation.
- Complete medicine name and its form: pill, injection or another type.
- Is it FDA-approved or made to order? Keep the official prescribing document for an approval claim, or the pharmacy’s written explanation for a made-to-order medicine.
- Who would make or supply it.
- The reason it is being proposed for my concern.
The bill and the next step
Keep the ongoing terms beside the quoted amount.
- What is included and what is extra.
- What renews and how to end it.
- How my plan’s answer affects the total.
- Who handles later questions.
Leave gaps visible
A blank space is useful. It tells you what still needs an answer.
- Circle the point that matters most to you.
- Keep the source link with the answer.
- Write the next question in your own words.
